Benefit Description
Group Health Insurance The provider is Health America, a Coventry Health Care Plan. Eligibility is the first day of the month following date of hire of full-time employment.
Dental Insurance Eligibility is the first day of the month following date of hire of full-time employment. MetLife is the provider.
Vision Insurance Eligibility is the first day of the month following date of hire of full-time or part-time employment. AccuSpec pays the full premium amount for employees. Highmark Fashion Focus Gold is the provider.
Life Insurance Eligibility is the first day of the month following date of hire of full-time employment. AccuSpec pays the full premium amount. The insurance is provided by MetLife.
Voluntary Supplemental Life Insurance This insurance is offered once a year. This benefit allows you to lock in affordable rates and is portable.
Accidental Death & Dismemberment Eligibility is the first day of the month following date of hire of full-time employment. AccuSpec pays the full premium amount.
Long Term Disability (LTD) Eligibility is the first day of the month following date of hire of full-time   employment. AccuSpec pays the full premium amount. LTD benefits, together with income from other sources, replace a percentage of an employee’s pre-disability income and may extend up to the employee’s normal retirement age. Metlife is the provider.
Flexible Spending Accounts (FSA) The FSA Medical Reimbursement Account allows you to set aside pre-tax earnings to be used to pay for eligible non-medical expenses during the year. Expenses include such items as major medical deductibles not covered by the HRA and co-insurance payments, dental bills, orthodontics, Lasik eye surgery, eyeglasses or contact lenses, hearing aids, routine physicals, prescriptions, etc.
401(k) Retirement Plan The 401(k) retirement plan is administered by MassMutual.  Eligibility is at date of hire. Company match eligibility is the first pay period following six months of employment. Employees may make pre-tax contributions and/or after tax (Roth) contributions, both up to the IRS limits. Rollover contributions from a previous employer’s qualified plan or regular IRA are permitted. Participants can choose from many different investment options. The company per pay period match is on a dollar for dollar basis, up to $600 per year. A three-year cliff vesting schedule applies.
Employee Assistance Program The Armstrong Employee Assistance Program (EAP) is available to all full time and part time employees within the Armstrong Group of Companies and their household members (includes children up to age 26 not living at home).  LifeSolutions (a UPMC work partner affiliate) administers the program at no cost to you. LifeSolutions is a professional and confidential resource designed to help you and members of your household, through coaching and counseling, life balance, work, and wellness.  Armstrong does not receive any personal identifying information about anyone who uses the program.
